SECTION 1: TERM

- Window Guards Notice: Pursuant to NYC Health Code § 131.15, Tenant must complete and return the Window Guard Questionnaire attached hereto. Window guards are required if children under the age of 10 reside in the apartment.
- Lead-Based Paint Disclosure: For housing built prior to 1978, the federal Lead-Based Paint Disclosure Form and EPA pamphlet are attached and incorporated herein.
- Notice of Rent Increase / Renewal Rights: Landlord shall provide written notice of non-renewal or rent increase greater than 5% pursuant to the mandatory notice timelines established by RPL § 226-c based on Tenant’s length of occupancy.
SECTION 5: UTILITIES AND SERVICES
- Landlord Provided: Landlord shall provide cold and hot water, and building structural maintenance.
- Tenant Provided: Tenant shall directly pay for all other utilities, including electricity, gas, internet, and cable, and shall establish accounts in Tenant's name prior to the Commencement Date.
SECTION 6: USE AND OCCUPANCY
- Permitted Use: The Premises shall be used strictly as a private residential dwelling for Tenant and the following authorized occupants only: [List Occupant Names].
- Subletting and Assignment: Pursuant to RPL § 226-b, Tenant shall not sublet or assign this Lease without the prior written consent of Landlord, which consent shall not be unreasonably withheld, subject to the procedures outlined in New York law.
