  1. Preparation: Open this document in Microsoft Word or Google Docs. Use the "Find and Replace" function to fill in all bracketed fields (e.g., [Full Legal Name of Parent], [State/Province]). Ensure no brackets remain.
  2. Review: Conduct a final clinical review. If the Principal has specific limitations (e.g., "Agent cannot sell the family home"), add those under Section 1(a) explicitly.

  4. Formal Execution:
    • Gather two (2) disinterested witnesses (not family members or beneficiaries).
    • Arrange for a Notary Public to be present.
    • Sign all copies in blue ink to distinguish originals from photocopies.
  5. Digital Execution: If using DocuSign or Adobe Sign, ensure the platform provides a "Certificate of Completion" and that your jurisdiction accepts electronic signatures for Powers of Attorney (most U.S. states do under UETA or E-SIGN, but some require wet-ink for real estate deeds).
  6. Record Retention & Filing:
    • Provide a certified copy to the Agent.
    • Provide copies to the Principal’s bank, investment advisor, and primary care physician.
    • Critical: If the Agent intends to manage real estate, the original Power of Attorney must generally be recorded with the County Recorder’s Office where the property is located.
  7. Version Control: Store the original in a fireproof safe. Retain the Document ID (POA-DUR-2024-INST-09) for reference in future amendments.
