Media Consent and Release Agreement

Instructions for Use

Operative Clauses

1. Grant of Rights: Subject hereby grants the Company, its successors, and assigns, the irrevocable, perpetual, worldwide, royalty-free right and license to use, reproduce, edit, exhibit, project, display, and publish the Media in any medium now known or hereafter developed, including but not limited to websites, social media, print advertisements, and broadcast.

2. Waiver of Review: Subject acknowledges that they have no right to inspect or approve the finished product, the advertising copy, or the printed matter that may be used in conjunction with the Media, or the use to which the Media may be applied.

3. Release of Liability: Subject hereby releases and discharges the Company from any and all claims, demands, or causes of action that Subject may now have or may hereafter have for libel, defamation, invasion of privacy, right of publicity, or any other claim arising out of or in connection with the use of the Media.

4. Compensation: Subject acknowledges that they are participating in this recording voluntarily and will receive [No Compensation / $__________] as full consideration for the rights granted herein.

5. Minor Consent (If Applicable): If the Subject is under the age of 18, the undersigned parent or legal guardian hereby consents to the terms of this Agreement on behalf of the minor.
