1. Parties and Definitions

2. Term and Services

3. Fees and Payment

4. Health and Vaccinations

5. Medical Emergencies

6. Behavioral Disclosure

The Owner certifies that the Pet(s) have not shown aggression toward humans or other animals. The Owner acknowledges that if the Pet(s) exhibit dangerous behavior, the Facility reserves the right to isolate the Pet(s) or require the Owner to arrange for immediate removal of the Pet(s).

7. Limitation of Liability

The Facility shall exercise reasonable care for the Pet(s). However, the Owner acknowledges that boarding involves inherent risks. The Facility is not liable for illness, injury, death, or escape of the Pet(s) unless caused by the gross negligence of the Facility.

8. Abandonment

If the Pet(s) are not picked up within [Number] days of the scheduled check-out date, and the Facility has received no communication from the Owner, the Pet(s) shall be considered abandoned. The Facility may, at its discretion, transfer the Pet(s) to a local animal control agency or rescue organization.
