Media Consent and Release Agreement

Operative Terms

1. Grant of Rights: The Subject hereby grants the Producer, its successors, licensees, and assigns, the irrevocable, perpetual, worldwide right to use, publish, reproduce, distribute, and display the Subject’s name, likeness, image, voice, and performance (collectively, the "Media") captured during the session occurring on [Date of Session] at [Location of Session].

2. Usage Scope: The Producer may use the Media in any medium now known or hereafter devised, including but not limited to websites, social media, marketing materials, television, film, and print publications, for any purpose, including commercial, promotional, or editorial use.

3. Waiver of Approval: The Subject acknowledges that they have no right to inspect or approve the finished product, the advertising copy, or the printed matter that may be used in connection with the Media. The Subject waives any right to royalties or other compensation arising from or related to the use of the Media.

4. Release of Liability: The Subject hereby releases and forever discharges the Producer, its employees, agents, and representatives from any and all claims, demands, or causes of action that the Subject may have arising out of or in connection with the use of the Media, including but not limited to claims for defamation, invasion of privacy, or right of publicity.

5. Representations: The Subject represents that they are at least 18 years of age and have the full right and authority to enter into this Agreement. If the Subject is under 18, a parent or legal guardian must sign below.

6. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the state of [State].
