• Completion: This form must be completed in full by the Service Facilitator and the Hiring Entity prior to the commencement of any services. All fields must be typed or printed legibly.
• Filing & Retention: Both parties shall maintain a fully executed original of this document in their respective corporate archives for a period of no less than seven (7) years following the termination of the service term.
• Mandatory Attachments: Ensure the following are attached as Exhibits: (A) Detailed Scope of Work/Service Specifications; (B) Fee Schedule & Payment Terms; (C) Insurance Certificates (if applicable).

• Compliance with all regulatory requirements applicable to their facilitation profession.

4. COMPENSATION & PAYMENT TERMS

The Company shall compensate the Facilitator based on the following structure:

5. CONFIDENTIALITY & NON-DISCLOSURE
