INSTRUCTIONS FOR USE

• Completion: Both parties (Service Provider and Client) must complete all bracketed fields. Ensure all schedules (Scope of Work/Fees) are signed and attached.
• Filing/Retention: Per the Companies Act 2016 and the Income Tax Act 1967 (Malaysia), this executed agreement and all relevant tax invoices must be retained for a minimum period of 7 years from the date of final performance.
• Attachments: Mandatory attachments include: (a) Detailed Scope of Work (SOW), (b) Fee Schedule, and (c) Copy of Business Registration (SSM) for both parties.
