Instructions for Use

Operative Clauses

1. Authorization. The Parent/Guardian hereby grants permission for the Student to participate in the off-campus activity described above. The Student will be under the supervision of School staff and authorized chaperones for the duration of the event.

2. Medical Authorization. In the event of an emergency, the Parent/Guardian authorizes School personnel to seek medical treatment for the Student. The Parent/Guardian agrees to be responsible for all costs associated with such medical care.

   • I have attached an updated list of the Student’s allergies and current medications.
   • The Student has no known allergies or medical conditions.

3. Assumption of Risk. The Parent/Guardian acknowledges that participation in off-campus activities involves inherent risks. The Parent/Guardian voluntarily assumes all risks associated with the Student’s participation, including travel to and from the destination.

4. Release and Waiver. To the fullest extent permitted by law, the Parent/Guardian releases and holds harmless the School, its employees, volunteers, and agents from any and all claims, demands, or causes of action arising out of the Student’s participation in the activity, except in cases of gross negligence or willful misconduct.

5. Code of Conduct. The Student is expected to follow all instructions provided by School staff and chaperones. Failure to comply with safety rules may result in the Student being removed from the activity at the Parent/Guardian’s expense.
