5. STEP-BY-STEP EXECUTION & FILING GUIDE

  1. Preparation of Document: Open this document in Microsoft Word or Google Docs. Use the "Find and Replace" function to locate all bracketed text (e.g., [Principal Address]) and input the specific factual data required.
  2. Verification of Authority: Ensure the Principal has a certified copy of the Minor’s birth certificate or a court order establishing guardianship to prove legal authority to delegate.
  3. Witnessing and Notarization: This document must be signed in the presence of a Notary Public. Most jurisdictions require the "wet ink" signature for POAs involving minors to be filed with schools or hospitals.
  4. Digital Execution: If using DocuSign or Adobe Sign, ensure the "Notary" module is activated (e.g., DocuSign Notary) to meet statutory requirements for remote online notarization (RON) where permitted.
  5. Distribution of Copies: Provide certified copies of the executed and notarized document to:
    • The Minor’s primary care physician/pediatrician.
    • The Minor’s school administration.
    • The Agent’s local hospital.
    • The Principal’s legal counsel.
  6. Record Retention: Maintain the original document in a fireproof safe. The Agent should carry a digital or physical copy at all times when traveling with the Minor.
  7. Expiration Monitoring: Set a calendar reminder for 30 days prior to the [End Date]. If the guardianship needs to be extended, a new instrument must be drafted and executed, as many states do not allow "automatic" renewals of parental power delegations.
