STEP-BY-STEP EXECUTION & FILING GUIDE

1. Preparation of Document

  • Field Completion: Open the document in Microsoft Word or Google Docs. Replace all bracketed text (e.g., [Principal Address], [Effective Date]) with the specific factual data.
  • Duration Check: Verify your local state laws regarding "Delegation of Parental Powers." If your state limits these to 6 months (like Michigan or Arizona), ensure the "Term" in Section 6.1 does not exceed that limit.

2. Signatory Verification

  • Ensure the Principal has valid, government-issued photo identification (Passport or Driver's License) ready for the Notary Public.
  • The Agent should also be present to sign the "Acceptance of Appointment" section if required by local practice, though the Principal's notarized signature is the primary requirement.

3. Formal Execution

  • Notarization: This document must be signed in the presence of a Notary Public to ensure enforceability with third-party institutions like hospitals and schools.
  • Digital Signature: If using DocuSign or Adobe Sign, ensure the platform provides a "Notary" add-on or that the Governing Jurisdiction recognizes remote online notarization (RON).

4. Distribution and Record Retention

  • Original: The Agent must keep the original notarized document.
  • Certified Copies: Provide copies to the Minor's primary care physician, school principal, and any travel carriers.
  • Digital Vault: Upload a scanned PDF of the executed document to a secure cloud storage (e.g., Dropbox, Box) accessible by both Principal and Agent.

5. Revocation Filing

  • If you choose to revoke this power before the expiration date, you must execute a formal "Revocation of Power of Attorney" and deliver it via Certified Mail to the Agent and all third parties (schools/doctors) who previously received the original POA.
