Terms and Conditions

1. Service Authorization: Owner authorizes Provider to perform grooming services including, but not limited to, bathing, brushing, clipping, nail trimming, and ear cleaning.

2. Health and Vaccinations: Owner certifies that the Pet is current on all required vaccinations, including [__________]. Owner must provide proof of rabies vaccination upon request.

3. Behavioral Disclosure: Owner warrants that the Pet has no history of biting, aggression, or unprovoked attacks. Owner agrees to disclose any known behavioral issues or anxieties the Pet may experience during grooming.

4. Medical Emergencies: In the event of a medical emergency, Provider is authorized to seek veterinary care at the nearest available facility. All costs associated with such care are the sole responsibility of the Owner.

5. Liability Waiver: Owner hereby releases Provider from any and all liability for injuries, illnesses, or accidents that may occur to the Pet during the grooming process, except in cases of gross negligence.

6. Pre-existing Conditions: Provider is not responsible for the aggravation of any pre-existing health conditions, including but not limited to skin sensitivities, heart conditions, or joint issues.

7. Cancellation Policy: Cancellations must be made at least [] hours in advance. Failure to provide timely notice may result in a fee of $[].
