INSTRUCTIONS FOR USE

• Completion: The Hiring Entity shall complete the bracketed fields in Sections 1–4. The Freelancer must review and verify all deliverables and payment terms before signing.
• Filing & Retention: Both parties shall retain a fully executed digital or physical copy of this document for a minimum of 7 years post-termination to satisfy statutory audit requirements.
• Mandatory Attachments: Ensure the following are appended: (A) Statement of Work (SOW), (B) Freelancer W-9 or Tax ID documentation, and (C) Exhibit of Proprietary Assets (if applicable).
